Helping Hand with Empty Sleeve

You can't offer a helping hand when you have nothing to give. Is the helping hand being offered because It doesn't cost anything when there is nothing to give or is it the thought that counts?

Also how difficult is it to offer help when the world is taking shots at you note (target and bullet holes).
